What SOAPsuds is and is not

SOAPsuds is an assistant that listens to a visit and writes a draft note for your review. It doesn't diagnose, refer, set urgency or make clinical decisions. You review, edit and sign every note.

What SOAPsuds does

  • Records an in-person or telehealth visit, or takes a recording you upload, and makes a transcript.
  • Writes a draft note in your template (SOAP, DAP or your own), using only what was said in the visit and what you typed.
  • Shows where each line came from, so you can check it against the transcript.
  • Offers optional tools, each labelled as a draft for your review: suggested codes, a documentation check, follow-up items and possible referral topics, and, where offered, conditions and plan options to consider.

What SOAPsuds does not do

  • It doesn’t diagnose, and it doesn’t tell you what to do for a patient.
  • It doesn’t make referrals or set how urgent something is. A possible referral topic is only a draft; nothing is added to the note unless you add it.
  • It doesn’t score or rank. The documentation check lists items to look at. It checks whether standard note parts are present and is not related to coding level or payment.
  • It doesn’t send anything to your EHR or to a patient by itself. You copy or export the note.
  • It doesn’t replace your judgment. Nothing it writes is final until you review it.

Your part

Every new note is marked AI draft · Not reviewed. Read it, fix anything that’s wrong by typing straight into the note, then mark it reviewed or sign it. Check numbers, sides (left and right) and who said what, because those are easy to mishear.

If you notice SOAPsuds wrote something that wasn’t said in the visit, edit the note and tell our team. See Something in the note is wrong.
